Patients

To register with Llansamlet Surgery we need your NHS number. This can be obtained from your medical card, a letter issued to you by the NHS or by simply asking your previous GP practice.  Once you have this please follow the following steps:

Print the registration form and relevant questionnaire (adult / child) and fill in the required details. Don't forget to sign the completed form.

Scan or photograph:

a). the completed paperwork;
proof of address - if over 18 years old (e.g. recent utility bill / statement)
c).identification (e.g. passport, driving license)
d).a copy of your last repeat prescription from your previous surgery (if you had medication on repeat)
 
Email everything to:

You will receive an e-mail to confirm that registration has been successful within 10 working days.
 
It is of the upmost importance that your information held at the practice remains current and correct at all times. 
You must inform us of any pertinent changes such as;
 
Change of Name.
Change of Address.
Change of Contact information such as your mobile telephone and or home telephone number.

 

Please notify the practice at the earliest opportunity to ensure your records details remain correct at all times.

If you are ill whilst away from your home or if you are not registered with a doctor currently but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

It's important for the practice to know if you are a carer for a close friend or relative.  This allows us to support you.  If this is the case, please register as a carer at reception.
